The phage is a direct contact mutagenic retro-virus.  It is programmed to be polymorphic and treatment resistant.  Victims transition through 3 phases.

The first phase is a lethargic zombie like state.  It does have increased healing and defenses.  Most infected either stay in this stage or die from it.  It is possible to treat this stage with advanced treatment antigens.

Phase 2, dexterity returns with a vengeance,  the body develops a chitinous armor and increase aggression and healing.  No turning back now 😉

Phase 3 takes on multiple forms depending on environment,  but all are elite combatants.

All phage have accelerated healing but are not immortal or undead in the horror sense.

It is thought that the phage was engineered to turn armies against themselves.

Also,  a small population on Earth were immune to the effects of the Phage.  They could still carry it but did not experience the symptoms.  It is believed the the majority of survivors on Earth during the rise of the Earth Defense Initiative and there descendants were carriers.  While those of the MSF had not been exposed.  It's also believed that the EDI spread the Phage to Apollyon and infected many of the MSF that arrived.

But there are also stories, that the Phage outbreak on Apollyon occurred before the arrival of the MSF. Perhaps the dormant infection mutated and became active again.  It's hard to tell.

According to MSF archives, when MSF arrived the EDI had already splintered into subgroups and signs of infighting had occurred.

So how did the phage manage to wipe out Earth? A war-torn world was probably armed to the freaking teeth. How deadly can the phage get? Like, how do you see them destroying some futuristic fortress city?

If the phage could wipe out Earth, why hasn't it wiped out the much weaker population of Apollyon?

Earth's population was close to 10 Billion when the Phage hit.   Earth was completely unprepared for it.  The original release was a an airborne carrier.  The carrier was designed to mutate into the virus over a 7 day incubation period. It spread worldwide before Earth ever new what hit.

What arrived first on Apollyon was the EDI,  a Military Organization with a high degree of genetic resistance to the Phage (due to population culling of the original outbreak).  Also,  the populations are smaller and more well armed limiting the spread.
